MHI Outreach Program

The Mended Hearts, Inc. has established an outreach program to reach underserved communities to establish local support and education for heart patients and their families. This program will use technology, mentorship programs, educational materials and training to connect historically marginalized individuals with Mended Hearts and the services we provide. 

Partnering with community centers, hospitals, cardiology offices, clinics, and places of worship, Mended Hearts is reaching out and connecting with patients and families and meeting them where they are. 

Training local leaders to provide support to others and provide educational resources to everyone in their community is a huge undertaking. We would love your support in helping others to build their local community support from the ground up.
