Mission
To eliminate health and social disparities in our community by finding those at risk, connecting them to care, and measuring the outcomes. We believe all communities can be transformed through the work of community health workers and the creation of community HUBs – an accountable care coordination delivery system.
About Us
CHAP serves as the Nationally Certified Pathways Community HUB for our community and multicounty region of Ohio. The HUB model is all about risk. It is about the comprehensive identification and reduction of risk. The HUB is also about building infrastructure for communities to be able to use resources more efficiently and effectively to address risk and improve outcomes.
Our mantra is “find – treat – measure”. Find the right people first, and then complete a comprehensive risk assessment. We look at everything that is getting in the way of good health outcomes – access to health care, housing, employment, safety, education, etc. – because it all matters. When risks are identified, they are then translated into Pathways. A Pathway is a tool developed to track one identified risk factor through to a measurable outcome. And finally, we measure everything that we do within the HUB.
CHAP partners with local agencies that employ Community Healthcare Workers (CHWs). The CHWs open the door for necessary services to reach clients, who are otherwise isolated due to cultural, geographic and economic barriers. Care coordination services do not duplicate, but facilitate and strengthen existing community resources. Through home/telephonic visits, the CHWs serve as an ongoing resource to neighborhood families, identifying individual needs and providing connections to appropriate services.
