A Unique Model for Community Health: Wesley combines a Federally Qualified Health Center (FQHC) and community center to holistically improve well-being.
- Mission: To make measurable health improvements through the provision of client-centered and client-guided programs and services, the reduction of health disparities, and the implementation of partnerships that can more holistically transform our communities.
- Services: Wesley provides comprehensive primary care (cancer screenings, child wellness, chronic disease management, OB/GYN, urgent care, etc.) to low-income, uninsured, and underinsured individuals of all ages, integrated with vital Community Center Programs and Youth Programs.
- Reach: Four Phoenix locations plus 2 mobile health units, extending access to underserved areas.
- Impact: Serves the highest percentage (70%) of uninsured patients among area FQHCs, demonstrating our commitment to those most in need.
- Community Programs: Enhancing well-being with two gyms, community gardens, after-school activities, adult education, exercise classes, and two fully licensed community kitchens.
