Dealing with a life-threatening illness, surgery, or a chronic disease can be overwhelming. Pathways has over 40 years’ experience providing care at home, giving patients the support they need to achieve their healthcare goals—maybe getting back to bridge at the senior center one day or being pain free to enjoy visitors more.

Pathways Hospice can’t change the fact that Lena M. is dying—she’s 83 and has end-stage heart disease. But we can do something about pain, about fear, about comfort, about joy, about grief. The specialized and sensitive care of hospice means better quality of life for Lena.

Our Home Health provides the nursing care and rehabilitation therapy needed to make patients independent in managing their own care. Pathways Home Health prevents hundreds of hospitalizations a year. We also have a unique Palliative Care program to manage symptoms for people like Bill C. He has terminal cancer and is receiving chemotherapy. Palliative care helps relieve the side effects of his treatment.
