Community Health Systems, Inc.- Fallbrook

Community Health Systems, Inc. (CHSI) is a nonprofit, 501(c)(3), Federally Qualified Health Center (FQHC), operating six community health centers in the tri-county area of: San Bernardino, Riverside, and San Diego. CHSI has provided uninterrupted services and has grown since 1984 from an average of 1,900 patients to more than 26,000 patients in 2019. CHSI's health centers provide primary and preventative medical care, women’s health services, behavioral health, dental care, vision care, health education services, and community outreach programs to all community residents regardless of their ability to pay. In response to the COVID-19 public health emergency, CHSI provided COVID-19 testing and vaccinations at all six health centers, including expanding and enhancing its telehealth service delivery model. 

Fallbrook Family Health Center (FFHC) located in North County San Diego, at 1328 S. Mission Rd., Fallbrook, CA 92028. CHSI became a recognized partner with Live Well San Diego on December 18, 2019. FFHC is the only FQHC providing an abundance of health care preventative services, and health educational programs within the Fallbrook region. FFHC assists community members to enroll into a health insurance plan, health program, and sliding-fee-scale. Moreover, FFHC continues collaborating and strengthening new relationships with Community-Based Organizations (CBOs) to mitigate the Social Determinants of Health (SDOH) and help achieve health equity. Through the support of CBOs, sponsorships, and monetary donations approximately 1,500 community members of Fallbrook and surrounding communities benefit from FFHC's free community health fair events: 1) Fallbrook Annual Health and Wellness Fair; 2) Fallbrook Backpack and Resource Fair Event; 3) Salvation Army Christmas Toy Distribution.
